COMMUNITY HEALTH WORKER:

Two full-time openings for Community Health Worker (CHW) for the Perinatal and Infant Community Health Collaborative serving Onondaga County. REACH CNY is a partner with Healthy Families, the Division of Maternal & Child Health of the Onondaga County Health Department.

CHWs assist clients to overcome barriers to health care and other needed services to improve women’s health, improve birth outcomes and infant health, and promote family health; and increase access to reproductive health services, health insurance and supportive services in the community.

CHWs provide services through outreach, home visits, and individual and group education sessions.

CHWs work collaboratively with Public Health Nurses, health care providers, and community agencies to provide health education, advocacy and supportive guidance to women. 

A valid driver's license and full time access to a vehicle are required.

The ideal candidate will be a peer of the women to be served, familiar with the Syracuse and Onondaga County community, and have an Associates’ Degree or a High School diploma (or HS equivalency with at least one year of related experience).

Proficiency in additional language(s) in addition to English a plus.

Starting wage approximately $20.65/hour. Excellent benefits.
